Making a Claim

All claims must be made by completing the Fund’s prescribed claim form, Claim Form B, which is available from the Fund offices, all major police stations, hospitals and selected post offices. The form is also available for downloading on this website under Making a claim. The completed form must be submitted with all the relevant supporting documents to any of the Fund Offices, either by hand or through the post. Claimants may collect the form from the above places and fill it on their own or they may seek assistance from the Fund or Post Office personnel at the selected Post Offices.

Necessary Documents

If the accident victim is injured:
•    Claim Form B
•    The doctor who first treated the claimant completes medical form Appendix B1
•    Police report, police statements, and police sketch plan
•    Documents to prove medical expenses e.g. medical bills
•    Documents to prove loss of earnings e.g. salary advise slip, tax returns.
•    Proof of guardianship of minor children
•    Proof of curatorship of adults with diminished mental capacity.
•    Copy of identity document
•    Copy of medical aid card if under medical aid
•    Proof of Residence/Work Permit for foreigners
NB: Under the new MVA Fund Act of 2007, any doctor who treated the claimant completes the medical form.


If the accident victim is deceased:
•    Claim Form B
•    Police report, police statements, and police sketch plan
•    Death certificate or post mortem report
•    Marriage certificate (if claim by spouse)
•    Birth certificate of minor children
•    Full proof of paternity in the case of children born out of wedlock
•    Proof of deceased earnings e.g. salary advise slip, tax returns
•    Proof of surviving spouses earnings e.g. salary advise slip, tax returns
•    Proof of funeral expenses e.g. receipts
•    Proof of guardianship of minor children
